I would like to use this conversation to float the idea of bringing back e-mail approval, unapproved registration, or to probe ideas for another type of registration that can be written into the forum software. I do not see the manual approval scheme which is currently being used as a good long-term solution so I ultimately plan on going back to something that does not require so much work on the administrator's end and would discourage fewer new users from joining.
Honestly the registration system we have really can't be improved. I've actually given this topic some thought myself and I couldn't come up with anything. The only possibility I suppose would be verification by phone. That in my opinion would be a little extreme and hard to implement, however.

An invite-based system where users who’ve already joined can invite new ones. That way abusive accounts can be traced to the one who’s responsible for inviting them.
